GUIDELINES AND PROCEDURES FOR ADMISSION

School Year 2019-2020


  1. Accomplished application form
  2. Photocopy (2 copies) of PSA-Authenticated Birth Certificate (please present the original copy for verification)
  3. Three (3) pieces identical and recent 2x2 colored picture, wearing white polo with collar, white background
  4. One (1) piece long plastic envelope
  5. Two (2) pieces long brown envelope
  6. One (1) piece long ordinary folder
  7. Assessment Test Schedule Form

  1. Secure an Application Form for Senior High School at Chinese General Hospital Colleges (CGHC) or download an application form from college’s website: cghc.edu.ph.
  2. Fill out an application payment slip at the CGHC Guard House.
  3. Pay the non-refundable assessment fee of two hundred pesos (Php 200.00) at the Cashier’s Office.
  4. Present the official receipt to the CGHC Guidance, Counseling and Testing Office and submit all necessary application requirements.
  5. Secure Assessment Test Schedule Form.
  6. Take the assessment test on the scheduled date. Examinees are requested to arrive 30 minutes earlier than the scheduled time. Late comers will be rescheduled.   

Proceed to the Guidance, Counseling and Testing Office for rescheduling.
Note:

  • Bring two (2) pencils and the Assessment Test Schedule Form.
  • The result of the assessment test will be posted on specific dates as scheduled/announced.


  1. Applicants who passed the entrance examination are required to pay a non-refundable and non-transferable fee of Php 2,000.00 (includes reservation fee of Php 1,000.00 and medical examination fee of Php 1,000.00) at the Cashier’s Office.
  2. The reservation fee will be deducted from the down payment upon enrolment.
  3. Successful applicants who failed to pay the reservation and medical examination fees on the dates given will forfeit their admission to the department and the slot will be given to other applicants.
  4. Reservations beyond/after the specified schedule period will depend on availability of slots.

Submit the following requirements at the Guidance, Counseling and Testing Office after completion of medical examination:

Incoming Students:

  • Original and photocopy of SF 9 (Report Card)
  • Original and photocopy of recent (dated on or after completion) Certificate of Good Moral Character with school seal
  • Original and photocopy of Certificate of Completion for Grade 10 Completers with the Learner Reference Number (LRN)
  • Original and photocopy of Voucher Certificate (if applicable)
  • Original and photocopy of Medical Examination Clearance issued by School Physician

 

Applicants with complete enrolment requirements may proceed to the 3rd Floor Student Activity Center for processing of ID.


  1. Fill up enrollment forms at the Guidance, Counseling and Testing Office.
  2. A Registration Form shall be issued upon submission of complete requirements for enrollment.
  3. Registration Forms will be given to Cashier’s Department for payment. Payment will be in the form of cash or manager’s check. Personal checks will not be accepted.
  4. After payment at the Cashier’s Department, Registration Forms will be given to the Registrar’s Office for enrollment confirmation and verification.
  5. Secure a copy of the uniform pattern and sample piece of cloth (swatch) at the Guidance, Counseling and Testing Office.
Note: In case of loss of Registration Form, secure a second copy from the Guidance, Counseling and Testing Office.
